Inspector's notes
Observed the cold holding well on the bowl making line to have TCS item that were at temperatures of 43* to 48*F. The items were raw tuna, cooked shrimp, cooked crab and raw tuna is sauce. Per PIC the items were placed in the unit less than 4 hours prior to inspection. Observed the cold holding well is designed to have ice to assist in maintaining temperature of the TCS items to be 41*F or less and no ice was present at time of inspection. PIC took the TCS items to rapidly cool and ice was added to the well at the time of inspection. Discussed with PIC except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Corrective Action: Corrected at time of inspection.

Discussed all ROP fish labeled "Keep Frozen" must be removed completely from all vacuum packages prior to thawing under refrigeration per manufacturer's specifications. All ROP fish was removed from ROP packaging at time of inspection. Also observed frozen shrimp sitting in water a sink not draining or with flowing water. Discussed with PIC all foods being thawed from frozen must be thawed in an approved manner. Foods must be thawed under refrigeration, under cold running water, or as part of the cooking process. PIC removed all fish from ROP packaging and drained the sink with the frozen shrimp and had added running water at time of inspection. Corrective Action: Corrected at time of inspection.

Observed raw ahi tuna, raw salmon, tofu, cooked chicken, cut lettuce, cut cabbage, and cooked pasta in the prep table with temperatures between 46 to 54*F at time of inspection. Observed tofu, cut kale in other prep table with temperatures between 54 to 59*F at time of inspection. Per employee and person in charge (PIC), all items had been recently stocked in the last hour except for the cooked chicken, cut cabbage, cut kale, and tofu. Cooked chicken, tofu, cut cale and cut cabbage were improperly cold held were discarded by PIC at time of inspection. All other TCS food items that were improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Discussed proper cold holding temperatures at time of inspection. Corrective action: Corrected at time of inspection.

Had employee re-sanitized glassware for 60 seconds. Observed Quat sanitizer with a concentration with 0 ppm at the 3-compartment sink. Employee remade quat sanitizer solution at time of inspection. Quat sanitizer solution concentration retested at ppm at time of inspection. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels.
