Inspector's notes
Observed oyster shell stock tags without the last date sold written on them. PIC stated that the establishment will no longer sell raw oysters. Current fresh oysters will be frozen and then cooked to serve to customers. All future oyster dishes will be cooked. Establishment will keep all previous shell stock tags on hand for the next 90 days. Shellstock tags must be kept with shellstock until sold, dated with final sale date, and maintained in chronological order for 90 days onsite. Corrective Action: Corrected at time of inspection.

There are still brass fittings on the downstream side of the RPZ backflow prevention device for the water supply line to the soda fountain carbonator. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. Replace all brass fittings and the brass angle stop on the downstream side of this device with plastic or stainless steel components.2. Repair the cracks in the cement floor of the walk-in unit room so that they are smooth, non-absorbent and easily cleanable.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. A coffee machine for customer coffee has been installed adjacent to the mop sink, chemicals and laundry machines in the laundry room. Beverage or food service cannot be conducted in the laundry room. This coffee machine must be removed from the laundry room.2. There are currently brass fittings on the downstream side of the RPZ backflow prevention device for the water supply line to the soda fountain carbonator. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. Replace all brass fittings and the brass angle stop on the downstream side of this device with plastic or stainless steel components.3. Provide microswitch activated and functional air-curtains for all three of the garage doors/windows that open to the outside in the bar area. The microswitches for the large dining room doors are not currently functional. The one for the bar window is functional.4. Secure the sinks in the bar to the floors or wall so that they are not loose and moveable. Metal braces can be used to secure the backsplashes of the sinks to the walls.5. Provide stainless steel caps for the stainless steel drainboard cabinets in the bar that do not have drain lines installed.6. Relocate the hand soap and paper towels in the kitchen so they are above the hand sink, not directly above the food prep sink.
