Inspector's notes
Observed an employee check phone, then touch face, then put on gloves and start touching bowls for customer orders. Observed another employee dry hands on pants, then continue touching food-contact surfaces. Discussed the importance of proper hand-washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Hands must be washed at any time contamination occurs and when changing tasks. Both employees properly washed hands at handwash sink at time of inspection and all food contact surfaces touched were placed in dishwashing area. Corrective Action: Corrected at time of inspection.

Inspector's notes
Observed pans of refried beans and roasted shucked corn in drawers of "chef base" style refrigerator in cook line with internal temperatures of 48-52 degrees Fahrenheit. Except during preparation, cooking, cooling or when time is used as a control all time-and-temperature control for safety (TCS) foods must be maintained at 41°F or below. Per person in charge (PIC), items were placed in unit at 9am, more than 4 hours prior to the start of the inspection. Ambient air temperature of the unit as measured with min-max thermometer was 55'F. Unit is not critical to cold-holding in the establishment. Unit may not be used to store TCS foods until repairs are completed and unit can hold cold at 41'F or lower. PIC voluntarily discarded items - see embargo form. Corrective Action: Corrected at time of inspection.

Observed tightly-covered containers of pico de gallo prepared at ambient temperature cooling in walk-in refrigerator. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. PIC removed lids during inspection. Salsas were within parameters for proper cooling times and temperatures. Corrective Action: Corrected at time of inspection.

Observed employee dredging raw chicken strips on cookline and place in fryer with tightly gloved hands. Employee then changed gloved using hands and handled RTE tortillas without washing hands. Had employee wash hands prior to continuing. Hands must be washed after handling raw animal proteins prior to donning new gloves when switching to a task where contamination could occur.
