Observed an employee removing a sticker from a container that was being put into a dishwasher. The employee was observed removing the sticker, placing it into the trashcan and then touching the inner rim of the trash can to detach the sticker from their gloved hand. The employee then resumed food prep. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed several dishes stored as clean at the clean dishes rack. Upon separating some dishes, several were observed with food residues and other greases still inside of the nested dishes. Also observed some clear nested containers with observable bits of food still stuck on the inside. PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. All food-contact surfaces must be cleaned to sight and touch. Final construction inspection conducted. Walk trough with the superintendent. Almost all items were corrected. Please continue with:1). Provide a Reduced Pressure Zone backflow valve under the prep sink for the water connection to the self cleaning ovens. Install with hub drain and lead drain to the floor sink. Valve will require testing and certification.2). Caulk around the front soda station drain line where leaving the wall.3). Provide a gasket under the dining room ice machine between the collar and the ice machine. Clean and seal.4). Please mount the base in the walk-in where open and caulk in at the floor juncture.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Pre-final walk through conducted. Store has hot water and refrigeration equipment is up and running, holding at proper temperatures. The following items were discussed for follow up before the final inspection:1). Please install micro switches on the drive through fly fans.2). Flash the sides of the soda counters, both west and east.3). Inside the west side SS cabinet near the custard machine caulk the cabinet vertically against the wall and horizontally inside and close the gaps.4). Please caulk the hand sink and dump sink splash guards in and outside at counter juncture.5). Install a rigid drain line. Lead the ice bin drain line through the wall, off the floor to the floor sink next to the dish washer. Properly brace, follow gravity flow and provide an air gap at the sink. 6). Provide a new rigid drain line for the soda station drain line from dining room soda station. See #5 for details.7). Install a drain line for the cold well next to the center hood and lead to the floor sink,8). Please flash off the back of the center hood, there is one panel missing.9). Near the office in the corner caulk the corner trim that is standing open.10). Provide a Reduced Pressure Zone backflow valve under the prep sink for the water connection to the self cleaning ovens. Install with hub drain and lead drain to the floor sink. Valve will require testing and certification.11). Install a Reduced Pressure Zone backflow valve upstream of the soda system at the connection. Hub drain can be led to the mop sink. Valve will require testing and certification. Do not install copper downstream.12). Please caulk any holes and gaps, examples are CO2 line next to fire box, low voltage line at east side drive through next to counter, electric conduit on top of the freezer and cooler leading to switch boxes, water heater platform edges and back edge, soda counter in dining room where silicone shrunk, etc.13). Please see if you can finish the tile work around the water and drain lines in the ladies room under the hand wash sinks.14). Provide a separate hot or cold water feed, cold or hot for the chemical dispenser at the mop sink.15). The drain line for the water filter over the mop sink will need to be re-run at the end, transition into a rigid line, brace and lead into the mop sink beyond the edge. Outlets always need to be led to the sink free from the edge and maintain an air gap.16). Please finish the wall penetrations around the soda lines under the counters, multiple and above the ice machine.17). Provide a dual check valve that meets plumbing standard ASSE 1022 upstream of the tea brewer. Just like the ice machine. Make sure to flush the water line first to avoid leaking. No pre-filter needed with RO water. Do not use copper or brass fittings.18). Caulk the gaps at the outside of the entrance of the walk in between the panels.19). One ceiling panel is damaged above the ice machine.20). Provide a gasket under the dining room ice machine between the collar and the ice machine.21). Please mount the base in the walk-in where open and caulk in.Store is okay to stock and train after city approval. No County legal action will result form this inspection.Printed on site.
