Observed purple urinal mat placed in men's restroom HVAC air vent. Ventilation was obstructed by a foreign object. Discussed with person in charge placement of a restroom item within the air vent creates a potential source of contamination through air circulation and should be used only for its intended purpose. PIC removed urinal mat from air vent. Physical facilities must be maintained in good repair and in proper use. Corrective Action: Corrected at time of inspection.
