E&D Class 4, 10+ seating.Establishment is approved for Other Effective Means of Barrier Protection.Correct the following items prior to your routine inspection:1) Provide a min/max thermometer or other approved device such as color-changing stickers for high temp dish machines.2) Provide a final version of the menu that has all items that may be raw or undercooked noted with an asterisk (*). 3) Secure drain lines under pre-rinse faucet so that they maintain a 1" air gap and do not need to be held together with plastic wrap. Plastic wrap was removed at time of inspection.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services.
