To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Sprayer may not go below the floor rim of the sink.(4) For Starbucks, have the ice machine and Reverse osmosis system delivered and installed.(5) At Murray cheese, provide 3/8 radius coved base within the kiosk, flash off the gap between the wall and the case(6) For Chicken/deli, have chemical dispenser installed at the mop sink.(7) Finish water connections to the combi ovens(8) Provide missing threshold at walk-in deli freezer(9) Finish out concrete curb at the hub drain located next to the deli walk-in cooler(10) Provide the threshold at the Bakery walk-in freezer(11) Install vent collars at the oven exhaust and escutcheon on the gas line penetration(12) Install the drain on the prep sink in the prep area. Equipment inspection.Construction is about 75% completeWalked site with Superintendent, FarrellAddress the following items:(1) Provide additional air curtains on both sets of doors leading to the covered patio area.(2) Fill in all cracks, saw cuts, expansion joints in the concrete floor at the front vestibule, inside the dock area walk-in coolers, all other areas where missed.(3) Install coved base where missing for the on-line order/staging area.(4) For the produce mist system, remove sections of copper after the backflow assembly and the tee section located at the produce case. Label the backflow(5) Sneeze guards may be required around murray cheese kiosk(6) Provide a spill-proof, pressure vacuum breaker for all department cleaning stations and at each mop sink.(7) Provide a double check backflow assembly (x2) on the Baxter ovens. Backflow to be looped down no higher that 5 feet above the floor for servicing (8) Flash off the gap between the proofer and the wall.(9) Flash off the gaps between the Baxter ovens.(9) Provide vent collars at the t-grid for the Baxter ovens (10) Ensure that all curbs have 3/8 cove where the curb meets the wall and the floor.(11) Have the RO system and ice machine installed at StarbucksPlease continue construction according to plans, stipulation letter, and inspections.
