Observed pan of spicy chicken patties with temperatures between 74 to 77*F on a speed rack at ambient temperature in the back of house at time of inspection. Per PIC and time stamp, spicy chicken patties were placed on the rack more than 2 hours prior to cool. Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with site superintendent Jerry V and RS #1292. Construction completed and approved. Inspection report was emailed to Jerry V.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with Site Superintendent Jerry V. At the time of this inspection, this project was about 85% complete. Inspection report was emailed to Jerry V. Correct/address the following items prior to final inspection:1) Remove all brass/copper fittings downstream of the RPZ backflow device and RO system. Or provide a water quality test that provides details of the total dissolved solids and PH of the water. 2) Remove all brass ASSE 1024 inline back flows for the non-carbonated beverage equipment. Provide a stainless steel ASSE 1022 dual check valve for each non-carbonated beverage equipment and the ice machines. 3) Relocate the air curtains above the walk-up windows so that they are positioned over the opening of the windows. 4) Provide an ASSE 1013 RPZ backflow device upstream of the misting system.5) Label each testable backflow device.6) Test and certify all backflow devices and provide the test report. 7) Shift the ice machine forward so that the leg does not sit on top of the floor sink. 8) Provide the C of O from the city. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company.
