Observed smoked tomato sauce with a temperature of 49*F in a covered container in the walk-in cooler. Per PIC, tomato sauce was made 3 hours prior to inspection. All TCS foods cooling were uncovered to allow for rapid cooling. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Provided PIC with cooling guidance documents at time of inspection. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with Eric (PIC). Construction completed and approved. ***Ceilings throughout the kitchen and bar must be maintained. Apply additional sealant when needed. ***Per Eric, RO system has plastic lines running through the brass shark bites. RO water does not service the kitchen or bar. ***Adhere to cleaning policy for honeycomb metal design within the bar. Inspection report was emailed to Eric.Correct the following items prior to routine inspection:1) Label each testable backflow device.2) Test and certify the PVB backflow device and provide the test report. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Conducted scheduled inspection with Eric. Inspection report emailed to Eric. ***A remodel permit will be required for the outside walk-in cooler.Correct the following items prior to final inspection:1) Relocate the RPZ backflow device in the walk-in cooler. Backflow device must be installed in a ventilated area outside of the walk-in cooler. Ensure RPZ is installed 12 inches from the finished floor and no higher than 5 feet. Re-test the backflow device and provide the test report. Provide a funnel drain below the relief port of the backflow device and drain to the floor sink.2) PVB backflow device must be installed 12 inches higher than the highest point downstream and must be mounted to the wall. Test and certify the backflow device and provide the test report. 3) Provide the C of O from the city of Mesa.4) Relocate the audio alarm for the sump pump so that it is visible. Ensure audio alarm is up off the floor at least 6 inches from the finished floor. 5) Provide 1-inch air gaps in between the end of all drain lines and the floor sink.6) Separate the jockey box drain line from the dump sink and drain to the floor sink. All drain lines have to be independent of one another. 7) Separate the dish machine drain line from the dump sink drain line and drain to a floor sink. All drain lines have to be independent of one another.8) Provide a 3/8” siliconized grout cove base within the walk-in cooler. 9) Silicone behind all sink fixtures. 10) Seal around the grease trap and provide handles for the two holes within the grease trap. 11) Seal around the sump pump cover.12) Provide additional information within the cleaning policy as discussed. 13) Trim around the foam within the restrooms and seal over the foam with a semi or high gloss paint light in color.14) Seal around all pipe penetrations throughout. No County legal action will result from this inspection.
