An additional application must be submitted for the Memory Care Service kitchen in order to be able to use this portion of the facility. If violations are not corrected by established timeframes, legal action may result. Re-inspection is scheduled for 02/26/2025.

The atmospheric vacuum breaker on the mop sink faucet in the Memory Care area is broken and leaking. Repair the atomspheric vacuum breaker on this faucet to be fully functional. Corrective Action: Correct prior to re-inspection. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: 1. There are currently long flex drain lines connected to the back of the ice machine in the Memory Care Service Kitchen area. Provide two, separate hard piped drain lines for this ice machine that empty indirectly into hub assemblies as discussed. Corrective Action: Correct prior to re-inspection.
