Inspector's notes

Observed no handwashing sink the facility at time of inspection. Per previous inspection, establishment had a handwashing sink next to the 3-compartment sink and mop sink at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Reinstall handwashing sink prior to re-inspection. Corrective Action: Correct prior to re-inspection on 6/11.

Observed a O-shaped toilet seat in the restroom at time of inspection. Provide a U-shaped toilet seat to the restroom prior to next inspection. Repeat violation: 2nd consecutive. Corrective Action: Correct prior to next routine inspection. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed leak at the 3-compartment sink faucet at time of inspection. Must repair all plumbing to a state of good repair prior to next routine inspection. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection

Observed no cove base at the floor and wall junctures in the back room and the restroom. Provide a 3/8" minimum cove to the floor/wall junctures of the back-room area and restroom. Recommend installing a rubber cove base. Repeat violation: 2nd consecutive. Corrective Action: Correct prior to next routine inspection 6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed buildup of food debris and dirt on the floor throughout the back room at time of inspection. Conduct a deep clean of the back-room area. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Repeat violation: 2nd consecutive. Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed multiple damaged and broken floor tiles in back of house at time of inspection. Replace the damaged floor tiles throughout the back area, as needed. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Repeat violation: 2nd consecutive. Corrective Action: Correct prior to next routine inspection.
