No bio-hazard spill kit in site yet. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. PAYMENT with a credit/debit card can be made on-line via the Permit Center using the invoice number at "Pay Invoice". Permit issued Eating and Drinking Class III 10+ Seating. Less than two items are cooked and cooled and reheated. Almost all items are prepped raw and cooked to order. For the oysters, keep tags with each lot, do not mix lots. Maintain tags for 90 days in order of harvest date.Please increase the light intensity in the walk-in with a brighter bulb, without food there as enough light but once filled I am afraid you will not make the 10 Foot Candles throughout at 3 Feet from the floor.This location does not serve sushi.

Flex line in place sags which is not approved. 2). Remove the water filter from the walk-in and fill in all the holes. Cap lines where needed. 3). Please install expandable plugs in the floor sinks at the bar. The bar will not be used. Corrective Action: Correct prior to next routine inspection.
