Owner/operator submitted a contract with BLT kitchen in Mesa which is not a commissary agreement. Submit a properly filled out Maricopa County commissary agreement form. I emailed the form to be completed and sent to me. Violation will be corrected upon receival.

MCEHC Chap. 8, Sec. 3, Reg. 5c - Mobile food establishments shall provide a commissary agreement to the Department upon request, at permit issuance or renewal, and as a part of the documents submitted during plan review for a new permit. Ice cream vendors that only sell or dispense individually packaged, commercially processed items are exempt from providing a commissary agreement.

The owner called me back and informed me that they just started operating in October. They mainly do events but will be at the bone house brewing in Fountain Hills from 5pm-8pm on Wednesday 12/13. Months of operation changed to reflect this info.
