Operating location provided at 3801 N 33rd Ave, Phoenix, Mon-Sun is not an Event, farmers market or other approved operating location within the limitations of the permit type. Also, the route sheet provided was blank. Provide a route sheet/operating location that is in compliance for your permit type. MCEHC Chap. 8, Sec. 3, Reg. 5b - Mobile food establishments shall operate from an approved commissary or other food service establishment as required by this Department, and shall report at least daily to such location for supplies, food storage, vehicle and equipment cleaning, waste disposal, and service operations. Addl Notes: Commissary agreement provided for Local First Community Kitchen has been closed out and is not valid. Provide a properly filled out commissary agreement for an approved facility with an issued permit.
