The digital menu had a consumer warning without an asterisk next to it at the bottom of the menu/sign. There were also a couple of items on the menu that were missing the asterisks. Person in charge (PIC) was able to update the digital menu at time of inspection. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Observed a buildup of dust and food debris on the ceiling tiles and vents above food prep and cooking areas. There is also a ceiling tile in the kitchen above the food prep that is not smooth and easily cleanable. Discussed with person in charge (PIC) the contamination risk is the debris falls over food. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.
