Inspector's notes
Observed chicken nuggets in hot holding area with internal temperatures of 106°F to 116°F that had been placed in unit two hours prior to inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. PIC chose to discard chicken nuggets. After discussion with the person in charge, it has been determined that the root cause of the violation was verification breakdown. Corrective Action: Corrected at time of inspection.

Observed the flooring near the ovens having the aggregate exposed. All floors must be smooth and easily cleanable. Also observed the sliding door of the deli cooler being in disrepair. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
