Inspector's notes
Hardboiled egg, shredded cheese, chopped lettuce, cottage cheese, and other TCS food items found on salad bar with internal temperatures of 48-51*F. PIC removed the food items from the salad bar and discarded them. These items were observed on one half of the salad bar and the other side was still functional maintaining temperatures of 41*F and below. Salsa bar observed with two types of salsa and jalapenos with internal temperatures of 46-49*F. PIC discarded the salsas. Coolers for the items mentioned above will not be used for cold holding TCS food items until they are repaired/replaced and able to maintain an ambient temperature of 41*F or below. Please ensure cold held TCS food items are maintained at 41*F or below. Corrective Action: Corrected at time of inspection.

-- Food employee observed using their bare hands to plate romaine lettuce for grab and go charcuterie snacks for patient line. Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. Requested at the time of inspection the employee rewash lettuce with fruit/vegetable wash treatment, wash their hands, and use suitable utensils or gloves before continuing to prep RTE foods. -- The water temperature for several hand wash sinks throughout the kitchen measured between 75°-77°F: (coffee shop, cafeteria grill, near pizza station, dishwashing area, by walk-in freezer, and near office). Hand wash sinks must be supplied with hot water of at least 100°F, soap, and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. At the time of inspection, a work order was placed and all hand sinks were repaired by maintenance. Corrective Action: Corrected at time of inspection.

-- In walk-in refrigerator, observed deep container of cooked ground beef from 3rd shift measuring between 47°F-55°F. Reviewed cooling and approved cooling methods with the person in charge. Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

The person in charge (PIC) stated the items were placed in the refrigerator less than an hour prior to the inspection. At the time of inspection, all TCS items were removed from the unit and placed in walk-in refrigerator to rapidly cool. Ensure TCS items are maintained at 41°F or less when cold holding.
