Observed accumulation of dust/debris along wire shelving units inside front of establishment walk-in cooler and along vent covers of walk-in cooler ceiling refrigeration units. Observed dust/debris along floors throughout establishment. Physical facility shall be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Establishment shall clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 2ND CONSECUTIVE VIOLATION. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed hole in wall next to establishment water heater. Observed rat bait/poison scattered throughout back of house area along floor and observed gallon container of Spectracide bug spray underneath table at front counter. All pest control for the establishment shall be performed by a certified pest control specialist and chemicals for pest control shall not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Instructed PIC to clean up and remove all rat bait/poison from premises prior to re-inspection. Corrective Action: Correct within 24 hours.

Observed rodent droppings on floor of dry storage room, along back wall of walk-in cooler, and behind establishment ice machine. Observed holes created by rodents along wall of dry storage room. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection.
