Observed entrance floor ramp panel in freezer walk-in #2 broken/missing peices and poking upwards. Also noticed floor panel near fans of walk-in unevenly protruding upwards. Discussed with PIC that All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable.
