Observed entrance floor ramp panel in freezer walk-in #2 broken/missing pieces and poking upwards. Also noticed floor panel near fans of walk-in unevenly protruding upwards. Also observed other walk-in door is peeling off from inside on lower right. Discussed with PIC that All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable.
