All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods -Observed cracked covering for soap dispenser in customer women's restroom. Soap is still able to dispensed. Also observed motion sense activated paper towel dispensers not working; person in charge will replace batteries. Paper towel roll was provided on sink counter top for use. Observed one stall missing a toilet paper roll dispenser; toilet paper roll was provided on top on trash can lid attached to stall wall. Discussed repairing/ replacing dispensers prior to next routine inspection and maintaining in good repair. Correct By: Correct Prior To Next Routine Inspection
