Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Establishment has added an automatic dishwasher in the bar and an RO system in the last few months. Per manager, a walk-in was added within the last two to three years. Additionally, there are waterline-connected coffee makers in the bar with no back flow preventors. All water connected, non-carbonated beverage dispensers must have an approved backflow device such as an inline dual check meeting ASSE 1022 standards.

Corrective Action: Corrected at time of inspection. In the top of the walk-in, mozzarella, cut lettuce, cooked chicken and cut tomaoes at 44*F to 47*F internally. The mozzarella had been in the unit more than two hours, per manager and was embargoed and discarded by the manager. The other TCS foods had been in the unit less than two hours and were moved to the walk-in to re-cool. Cooling sausages and chicken had been placed in the lower section of the unit. Manager removed cooling items to the walk-in.

All ROP fish labeled "Keep Frozen" must be removed completely from reduced oxygen packaging (ROP) prior to thawing under refrigeration or immediately after thawing under cold running water. To ensure food safety, prevent physical contamination and the transmission of disease; employee eating, drinking, beverage storage, and using tobacco must be done in an approved area. Observed a sheet pan full of roasted chicken on a cooling rack near the washer and dryer with an internal temperature of 78*F with probe thermometer. The Person in Charge (PIC) was not sure how long the chicken had been sitting out. PIC discarded the chicken at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below.
