Upon re-inspection, all violations due for correction have been corrected. Observed pests have been removed and floor has been cleared.
Inspector's notes
Observed various food debris and garbage on floor of back storage room. Discussed with person in charge importance of removing food debris and potential pest harborage items from back room. Observed massive ice buildup in walk in freezer taking up a quarter of the space. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed broken fan in walk in freezer. Observed one fan in operation, one fan struggling to spin, and one fan completely detached sitting on guard. All items were observed still frozen. Discussed with person in charge repairing fan as to not overwork and damage the unit. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed evidence of pest activity. Observed multiple sticky traps in backroom storage area containing numerous pests including crickets, cockroaches, spiders, and a lizard. Observed dead crickets in walk in cooler and a cockroach in the men's restroom. Reviewed pest control invoices with person in charge, last visit being 1/11/2026. Scheduled return in 9 days with person in charge to observe progress with pest cleanup and control. Establishment must be maintained free of pests, and all pest control measures must be conducted by a licensed pest control specialist. Corrective Action: Correct prior to re-inspection.
