Observed cooked chicharron with meat with no time stamp at time of inspection. Person in charge (PIC) verified these items are under time as a control at time of inspection and confirmed items were made an hour before inspection. Employee had placed tag with date and no time. Explained to PIC that time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Items should not exceed over 4 hours based on time as a control policy. All TCS foods being held under time as a control were properly time stamped at time of inspection. Corrective Action: Corrected at time of inspection.

PIC confirmed they did not have a consumer advisory and confirmed the shrimp ceviche is only marinated in lime. Explained to PIC that offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Provided PIC a copy of a consumer advisory and added asterisk at time of inspection. Corrective Action: Corrected at time of inspection.

Observed multiple spoons and tongs used to serve food in the hot line in a bucket with water at a temperature of 72*F at time of inspection. Explained to PIC that in-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Observed a pan of cilantro bunches next to raw drying pork skin. The left side cilantro bunches were touching the raw hanging pork skin in food prep area. Cilantro was discarded by manager. Observed a rack of hanging pork skin where the bottom 8 pork skins were touching the floor in the prep area. The bottom 8 pork skins were discarded by the manager. Observed 5 boxes of raw shelled eggs stored above bakery goods such as filling and biscuits. Also observed raw prepped shrimp stored above ready to eat diced tomatoes. Also observed 11 pans of prepped raw chicken stored above multiple pans of cooked pork. PIC reorganized food so that eggs were below bakery goods. raw shrimp was on the bottom shelf in it's own area, and chicken was moved to its own area as well. Food chart handout was provided. Ensure that food is organized in a manner that prevents cross contamination following the food storage chart provided. Correct By: Corrected At Time Of Inspection
