PRIORITY VIOLATION-5-202.11 (A) - Priority: Approved System and Cleanable Fixtures; According to Law Predefined Comment: PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Predefined Comment: Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. PRIORITY VIOLATION-5-202.14 - Priority: Backflow Prevention Device, Design Standard Addl Notes: Observed missing cover for the atmospheric vacuum breaker at the mop sink. Showed manager a print out of an atmospheric vacuum breaker with included cap. Ensure that cover is replaced, and send picture of fully functioning mop sink within 3 days. Corrective Action: Correct prior to re-inspection. Facility sent emailed picture of obtained cover for AVB supplied mop sink. All items have been corrected.
