Inspector's notes

Observed the prep sink by the rice cooker with constant dripping at the time of inspection. Also observed the handwashing sink in the back disconnected at the base and leaking onto the floor. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. PIC has a work order in for the handwashing sink and is putting one in for the prep sink faucet. Corrective Action: Correct prior to next routine inspection. PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines Addl Notes: Observed chemical dispenser downstream of AVB under pressure at the time of inspection connected to the chemical dispenser by the mop sink. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Instructed PIC to unscrew the hose downstream of AVB when not in use at the mop sink and three compartment sink. Corrective Action: Corrected at time of inspection. Second consecutive repeat violation.

Observed in hot box in back of house container of cooked brown rice at 95*F, as measured internally with a probe thermometer. Per PIC, rice had been in the unit for 30 minutes. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. PIC chose to discard improperly hot held rice and increased temperature of hot box. Corrective Action: Corrected at time of inspection.

A plumber should be called out to ensure that there aren't any issues which could lead to backflow. The ice scooper is stored on the wall where it could be bumped into, if possible move the scooper to a holder with a lid or store in the ice ensuring that the handle remains out of the ice. The current spot could result in contamination from hair or food.

Inspector's notes

Observed cooked steak in tray of hot holding unit to be between 110- 120*F , an employee informed me that the steak had been pulled off the grill fifteen minutes earlier and was about to be cut up. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. PIC put steak back on the grill ,all TCS foods improperly hot held were reheated at time of inspection. Corrective Action: Corrected at time of inspection.
