Observed cooked rice cooling in a very large pot that was tightly covered with a lid. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods being cooled in deep containers were transferred to shallow containers to allow for rapid cooling. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed multiple TCS items (such as chile rellenos, cooked chicken, and cooked beef) with internal temperatures between 48-55°F at the time of inspection. Two refrigeration units were down with ambient air temperatures of 48°F and 58°F. The Person in Charge (PIC) stated that the temperatures of the refrigerators were not monitored at all today, so all TCS foods improperly cold held were discarded by PIC at time of inspection. Refrigeration is adequate. These units will not be used until they are repaired. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Correct prior to next routine inspection.

Food in these units are discarded every 3 hours. Discussed TPHC procedures and documentation requirements with the Person in Charge (PIC) at the time of inspection. Written procedures for TPHC must be on-site at all times. When following TPHC ensure the proper procedures are followed: initial start temperature for 4 hour hold time is 41°F or less when removed from cold holding and 135°F or greater when removed from hot holding, food is to be cooked and served within 4 hours from time it was removed from temperature control, food has to be marked or otherwise identified to indicate the time that is 4 hours past the point the TCS food was removed from temperature control, and food that is unmarked or marked to exceed 4 hours must be discarded.
