Observed multiple spray bottles containing chemicals without labeling throughout the establishment. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Bottles were labeled with marker at the time of inspection. Corrective Action: Corrected at time of inspection. PRIORITY VIOLATION-7-204.11 - Priority: Sanitizers, Criteria-Chemicals Addl Notes: Observed chlorine concentrations above 200ppm in the three compartment sink. Sanitizer was re-made and test strips were used to confirm the concentration at the time of inspection. Discussed importance of sanitizer concentration and testing with employees and the Person in Charge. Ensure all chlorine sanitizer concentrations are between 50-100ppm at all times. Corrective action: Corrected at time of inspection.

Corrective Action: Correct prior to next routine inspection. 6-101.11 - Core: Surface Characteristics-Indoor Areas Addl Notes: Observed metal plating separating from the wall, and large missing pieces of the floor that were holding standing water. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.
