-- Insect fly light observed directly under pizza/bread/dessert boxes and wing bowls. Some of the boxes were face up. No visible contamination observed. Discussed approved design criteria for pest control devices with the PIC: Devices used to electrocute or stun flying insects must be designed to retain the insects within the device, the device must not be installed directly over food preparation areas, and they must be positioned so that dead insects and insect fragments are prevented from falling or being propelled onto exposed food, food contact surfaces, equipment, linens, and unwrapped single-service items. The person in charge stored all items face down at the time of inspection. Corrective Action: Corrected at time of inspection.

Small holes in wall also present in ware wash area. Coved base missing below pizza boxing station, separating in women's restroom and behind hot box. Please repair/resurface these areas to provide smooth and easily cleanable surfaces prior to next routine inspection ***2nd Consecutive Violation*** Core-6-501.12, C: Cleaning, Frequency and Restrictions - Green storage racks throughout facility observe with debris or dust build up notably above the pizza boxing station.
