Observed roasted tomato sauce in a deep container and with seran wrap in walk-in with internal temp of 47*F. PIC stated that it had been prepared and placed in the walk-in at 8:30AM. PIC moved sauce into freezer to cool it down, after rechecking the temp was at 45*F. 4 hours had passed from start of cooling process by that point so all TCS foods improperly cooled were discarded by PIC at time of inspection. TCS foods prepared from cold or ambient room temperature ingredients must be cooled from 70°F to 41°F within 4 hours. Advised PIC to use shallower containers and metal containers to ensure proper cooling. Corrective Action: Corrected at time of inspection.

Observed corn in reach-in cooler in a deep plastic container with an internal temp of 71*F. Employee stated it had been placed in there from a can 1 hour prior. Advised employee to cool foods in walk-in rather than reach-in cooler. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods being cooled in deep containers were transferred to shallow containers to allow for rapid cooling. PIC moved corn to freezer in a metal container to allow for rapid cooling. Corn was later observed at 48*F and dropping. Corrective Action: Corrected at time of inspection.

Observed employee handle raw chicken and then wash hands for approximately 5 seconds at time of inspection. Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Observed cooked bamboo, partially boiled eggs, and cooked mushrooms in metal pan with water over small stovetop on cook line at 96*F-133*F. Per PIC, food items were placed in unit at approximately 11:30AM. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. When reheating TCS foods from refrigeration to be hot held, other TCS foods (not from a commercially packaged source) must be reheated to at least 165*F, while cooked plant foods shall be reheated to at least 135*F. Discussed adding more water to steam tray to ensure water level reaches level of food in containers with PIC. All TCS foods improperly hot held were discarded by PIC at time of inspection. Observed pre-cooked sliced pork portioned in multiple containers in walk-in refrigerator at 47*F-53*F. Containers of pork were observed to be tightly covered with plastic wrap and stacked on top of each other. Per person-in-charge (PIC), pork is received pre-cooked, kept in walk-in refrigerator, sliced and portioned, and placed back in walk-in refrigerator to cool to 41*F. Per PIC, pork was sliced and portioned 3.5 hours prior. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. PIC single-stacked and uncovered containers and added ice bags around containers at time of inspection. Pork was observed to be at 43*F within 30 minutes. Observed cooked chicken broth with internal temperatures between 44*F-51*F; they were stored in five 18-quart plastic containers with the lids still on. Person-in-charge (PIC) indicated they were cooked and transferred into containers yesterday. PIC stated ice wands and ice baths are utilized to cool cooked broth down to temperature. Cooked time/temperature controlled for safety foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Discussed acceptable methods for properly cooling TCS foods, which include: placing food in shallow pans no more than 4 inches, separating the food into smaller or thinner portions, using rapid cooling equipment (i.e. a blast chiller), stirring the food in a container placed in an ice water bath, using containers that facilitate heat transfer (such as metal), and adding ice as an ingredient. Discussed utilizing smaller containers during cooling and transferring to larger containers once TCS food is properly cooled. Person in charge indicated they started cooling around 10:30 am (~2.5 hours before inspection). Cooked time/temperature controlled for safety foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. The cooling methods and cooling guidelines guidance documents were emailed. Person in charge was informed that kimchi is not exempt from date marking; the lists of exemptions were emailed to the person in charge. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Person in charge indicated they ran out of the labels for these containers; person in charge was informed that in lieu of labels they can write the working names of these chemicals on tape and affix to the container or write it directly on the container. Working containers used for storing poisonous/toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material to help prevent the dangerous misuse of the contents. Final construction inspection for a Class 4 10+ E&D permit was conducted with Tom S. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Construction inspection for a Class 4 10+ E&D permit was conducted with superintendent Ralph H. by appointment. The project is approximately 95% complete. The establishment is approved to stock and train. Please ensure the following items are addressed by the completion of the project: 1) Provide hot water measuring 100' F minimum to the restroom handwash sinks. 2) Install an air curtain at the window opening located at the bar. The air curtain must be operated by a micro-switch. No manual switches can operate the air curtain unit. 3) Trim or lift the indirect drain lines for the pasta cooker and bar drains to ensure 1" minimum air gaps are provided above the floor sink. 4) Provide the covers for the large wall holes and caulk all wall holes throughout the facility, as needed.5) Paint the wall to the left of the three sink with a semi-gloss paint finish. 6) Provide all missing dome grates at the floor sinks, as needed. 7) Caulk-in all escutcheon plates and plumbing penetrations with silicone. 8) Provide all missing cleanout covers. 9) Provide concrete sealer around the concrete area around the bar floor sink. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. Please ensure the following items are addressed by the completion of the project: 1) Provide a minimum 75 gallon capacity water heater unit. The 50 gallon high recovery unit is not approved. 2) Install an air curtain at the window opening located at the bar. The air curtain must be operated by a micro-switch. No manual switches can operate the air curtain unit. 3) Observed the cove tile at the walk-in cooler top set. Provide a 3/8th inch minimum cove to the walk-in cooler interior by utilizing a silicone grout. 4) Provide a level 4 finish to the brick wall areas in the back kitchen area. Ensure a semi-gloss paint is utilized. 5) Provide a semi-gloss paint to the exposed dry wall areas at the front service area. 6) Ensure u-shaped toilet seats are provided in the restrooms. 7) Provide sanitary napkin waste containers in each restroom unit. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below.
