The establishment a large functioning dishwasher they can wash, rinse, and sanitize in with plumbing in good repair. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to re-inspection on 08/02/24.

OC re-inspection for a Class 3 10+ E&D permit was conducted with manager Miguel T. by appointment. The establishment is still approved to operate. Please address the listed items prior to the next routine inspection: 1) The y-valve was disconnected from the mop sink faucet at time of inspection. Please provide a quick disconnect to the mop sink faucet if the facility wanted to keep utilizing the sprayer hose. The current handwash sink installation for the cook line and warewash area is not ideal based on location. It is recommended to shift the sink to the left approximately 3-4 feet to provide a more central and easily visible location for both areas. If handwash violations are noted at the cook line in the future then the sink will need to be shifted or an additional handwash sink will need to be installed. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

OC inspection for a Class 3 10+ E&D permit was conducted with manager Miguel T. by appointment. The establishment was close at time of inspection, but is currently operational. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. PRIORITY VIOLATION-5-202.14, P: Backflow Prevention Device, Design Standard ------- Observed a pressurized sprayer hose installed at the mop sink with an atmospheric vacuum breaker installed. The sprayer pressure was released at time of inspection and the sprayer was removed at time of inspection. The PIC was advised to install a quick disconnect for the sprayer and to release the water pressure in the hose after every use to prevent damaging the AVB.
