Observed employee touch hair and hat before bagging pita bread without washing hands prior. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Observed mechanical ware washing machine leaking large amounts of water onto the floor when in use and pooling near the back wall due to slanted floor. Informed PIC that the machine should not be used until it is brought back into good repair. Discussed the importance of proper machine maintenance, preventing pooled water, and preventing water damage to floors and walls with PIC. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Also discussed either moving chemical dispensers or dishwasher so the drainboard where dirty dishes are stored is not directly above the dishwasher where clean dishes maybe stored (dishwasher was not in use at time of inspection). Person in charge indicated that the sanitizer and soap dispenser were switched which resulted in changing which drainboard was directly above the dishwasher.

The sanitizer in the compartment had a measured concentration of 0 ppm quaternary ammonium. The person in charge drained the compartment, returned the bowl to the wash compartment, and remade the sanitizer solution; the new solution had a concentration of 200 ppm. Discussed with person in charge ensuring employees are measuring the solution using the manufacturers instructions (i.e. do not shake the test strips in solution). Quaternary ammonium sanitizer must have a concentration of 150 - 400 ppm; anything lower than 150 ppm may not be effective for cleaning and anything above 400 ppm may leave toxic residuals on food contact surfaces and equipment. PRIORITY VIOLATION-3-501.16(A)(2) and (B), P: Time/Temperature Control for Safety Food, Cold Holding. The measured ambient temperature of the unit using a min/max thermometer was 60.8*F. The person in charge indicated these mixes had only been in the unit for less than hour and voluntarily discarded them for quality purposes. Also observed garlic sauce (garlic in oil) and yogurt sauce with internal temperatures of 50*F in squeeze bottles. The bottles are kept on the prep line but are not completely submerged in the unit or are in metal pans inside of refrigerated prep units; all other foods in theses units are 41*F and below. Discussed time as a control as a possible solution to holding time/temperature controlled for safety foods in squeeze bottles outside of the walk-in cooler/reach in refrigerator units. Person in charge willingly discarded of the squeeze bottles of garlic sauce and yogurt sauce where time and temperature had not been tracked and put all other bottles that had been out for less than 45 minutes back into walk-in refrigerator to cool back down to 41*F and below. Except during preparation, cooking, cooling, or when time is used as a control all Time/Temperature Controlled for Safety foods MUST be maintained at 41°F or less OR 135*F or more. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or less. Correct By: Corrected At Time Of Inspection

OC inspection for a Class 4 10+ E&D permit was conducted with superintendent by appointment. The establishment was closed and not operating at time of inspection.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department.
