Final construction inspection for the Retail Grocery Permit. Construction complete and approved. Superintendent will follow up on the items below before turning over the store:1). Soda drip tray drains will be installed and led to the floor sink.2). The drip tray for the spray down hose under the soda station will be installed and plumbed in.3). All walk-in doors will be adjusted and self closing.4). The penetrations around the sprinklers have a boot but will still be foamed in as well.5). Stainless panels will be installed over the ice bins next t the ice machines on the soda station. Make sure they cover the whole opening and are cut to size to fit.6). Backflow test will be conducted today and reports will be sent by e-mail.7). Debris in ice bins need to be removed and first ice batches dumped.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. Second walk through of the retail permit with the superintendent. Store is 85% completion. The following items were discussed:1). Walk in penetrations will be filled in, for all penetrations, electric, condensate and sprinklers.2). All partition walls and center island will be based out with coved base.3). Please install a splash guard, right side of the south front kitchen hand wash sink.4). Door frames inside the walk-in coolers should be caulked in where not tight to avoid mold growth. 5). Eliason door door frames will be finished out between back room and kitchen and south side walk in cooler.6). Where cooler panels were cut the edges and material will be finished out.7). Ice machine plumbing will be finished up.8). Air curtain will be mounted over both doors if it is full width. If not, install over the largest man door only.9). At the mop sink, caulk the edges of the escutcheon plates to keep water out of the wall.10). Ice bins for soda stations will be cleaned out and properly fitting lids installed.11). Caulk the hood on the under side stainless wall panel joint with silver caulk.12). Grout the floor sink in under the three compartment sink.13). Finish out wall penetrations in the "lactation" room- former ice machine room.If you want, add a shut off valve to the discharge line of the sump pump. Not required.At final, all equipment will need to be in place, refrigeration equipment on and holding temperatures, hot water, all backflow valves (that are testable) need to be tested.No County legal action will result from this inspection.E-mailed to superintendent.

First walk through of the Quik Trip #1426 with the superintendent. (Retail Grocery Permit) The following items were discussed:1). Floor tile, concrete will be grouted to the wall throughout before the metal or vinyl base is installed. 2). All penetrations through the walk-in walls and ceiling will be sealed including inside the electric conduit where possible to stop moist air migration.3). The following backflow valves will need to be installed:a). Reduced Pressure Zone backflow valve (ASSE1013) for the Multiplex #509.b). Reduced Pressure Zone backflow valve (ASSE1013) for the spray down hoses in the coffee counters. One for each side Detail A1 and H9, page P512 #803 .c). Reduced Pressure Zone backflow valve (ASSE1013) water feed to the oven feeding the self clean cycle.d). Reduced Pressure Zone backflow valve (ASSE1013) under the rinse sink in the kitchen feeding the in sink spray wand P600 #613.e). Reduced Pressure Zone backflow valve (ASSE1013) back room, upstream of the RO system.f). Reduced Pressure Zone backflow valve (ASSE1013) back room water line to the pressure wash system.4). All non-carbonated beverage machines will receive dual check valves that meet plumbing standard ASSE 1022.Make sure to flush lines before installing the valves or remove, flush and re-install. Many if the check valves are sensitive to debris.5). Back room will also be based out.6). Please install coved metal base around the center island in the kitchen.7). Please install a splash guard on the right side of the front kitchen south side hand sink.8). The dish washer drain line will need to transition into a rigid pipe if not planned.No County legal action will result from this inspection.E-mailed report.
