Observed nozzle attached to mop sink hose downstream from Atmospheric Vacuum Breaker (AVB) at time of inspection. Discussed removing nozzle or hose with PIC when not in use to prevent damage or failure of back flow device. PIC had nozzle removed at time of inspection. Corrective Action: Corrected at time of inspection.

Final construction inspection walk though. Construction complete and approved.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. First inspection walk through. Construction is at about 80% completion. Plumbing is in place and complete. Sinks are being set and base tile is finished up. The following items are planned:1). Mop sink will be caulked in at all junctures.2). Walk-in penetrations will be filled in, like sprinklers and electric.3). After base tile is set, make sure to grout and gaps under the tile and make sure that you keep the joint beveled in line with the tile to retain the cove.4). Front counter will be finished out and caulked.5). Back door will be insect tight.6).
