Discussed permiting process with operators.Final Construction inspection conducted with person in charge, Gabe. Confirmed permit type with PIC. Construction completed and approved. Inspection report was emailed to Gabe.Prior to first routine inspection:1. Provide a second coat of semi or high glass paint in restroom.2. Continue to seal flashing in walk-in and at mop sink.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with manager, Gabe.At the time of this inspection, this project was about 70% complete. Inspection report was emailed to Gabe.Prior to final:1. Provide a NSF approved hand wash sink with hot and cold water in the area of the ice machine. Discussed moving ice machine to an area that already has a hand wash sink that is within 25 feet, visible and easily accessible at all times.2. Wall surrounding ice machine will need to be water resistant (FRP, tile, stainless).3. Flash gaps between wall junctures at new mop sink with water resistant material.4. Provide faucet at 3 compartment sink that can fill all compartment. Currently only a spray faucet is attached.5. Repair leak at prep sink.6. Provide hot water valve handle at new mop sink.7. Seal flashing on walk-in with silicone.8. Provide self closer on door on employee restroom.9. Paint any repaired dry wall with semi or high gloss paint.10. If using FOH area for food prep, then: Install cove base along all walls, Remove laminate counter top, repaint ceiling with a light colored high gloss paint, and repair hole in ceiling.11. Provide cove base around mop basin.If using wood burning stove, ensure that city has signed off on use.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No county legal action will result from the inspection.
