Observed sprayer nozzle with shut off installed on the mop sink downstream from an atmospheric vacuum breaker (AVB). Person in charge (PIC) removed y-valve at the time of inspection. Discussed the importance of keeping y-valve removed from the mop sink to ensure the AVB backflow preventer remains functional. Shutoff valves are not permitted downstream from an AVB backflow preventer. Corrective action: Corrected at time of inspection.

Final construction inspection for a Class 2 10+ E&D permit was conducted with owner Kristin B. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Construction inspection for a Class 2 10+ E&D permit was conducted with superintendent Armando M. by appointment. The establishment was closed and not operating at time of inspection. The permit is not approved to operate. The permit is approved to stock and train. Please address the following items prior to the final construction inspection: 1) Provide hot water measuring 120' F minimum to the plumbing system. 2) Provide a sneeze guard along the service counter, as discussed. 3) Fill the gap around the three sink floor sink to provide a smooth and easily cleanable finish. 4) Provide all missing dome grates at the floor sinks. 5) Provide a covered sanitary waste container in the restroom. Equipment inspection for a Class 2 10+ E&D permit was conducted without inspection contact present. Two electricians were present at time of inspection. The project is approximately 80% complete. Please ensure the following items are addressed by the completion of project: 1) Provide a splash guard to the right of the prep sink. The splash guard must match the length of the sink and the height of the faucet. 2) Provide a 3/8th inch minimum cove base to the floor/wall junctures of the restoom. 3) The painted wall areas of the front service and restroom must be semi-gloss minimum. Observed an egg shell finish. 4) Caulk the countertop/wall gaps at the front service area with silicone. 5) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. 6) Ensure all fixed wall shelves and sink fixtures are caulked to the walls. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
