Observed no hand soap at the handwashing sink in the back of house at time of inspection. Person in charge (PIC) provided soap for handwash sink at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Inspector's notes
Observed raw chicken in bottom shelf of refrigerator and asked how it was prepped. Per PIC the raw chicken is taken to the personal appartment above the facility to marinade and brought back to the facility. Also observed purple cooked rice and asked about prepping process. Per PIC the purple rice is made at the Operators personal residence and brought in to sell. Embargoed both the raw chicken and the purple rice and PIC discarded at time of inspection. Discussed with PIC all food must be obtained from an approved source as defined by law. Corrective Action: Corrected at time of inspection.

Final construction inspection for a Class 2 10+ E&D permit was conducted with owner Colden N. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Construction inspection for a Class 2 10+ E&D permit was conducted with superintendent Sam Y by appointment. The project is approximately 95% complete. Please ensure the following items are addressed by the completion of the project: 1) Install an ASSE 1013 reduced pressure backflow preventer of the RO system. Ensure no copper or copper alloys are downstream of the backflow. Certify and provide a test report. 2) Install an ASSE 1013 reduced pressure backflow preventers upstream of the glass rinser and pitcher rinser at dump sink. The hot and cold water supplies at the dump sink will each need a backflow. If RO water is used for the rinser then the backflow must be stainless. Certify and provide a test report. 4) Provide a sneeze guard around the front service ice well, as discussed. 5) Provide rubber cove base to the millwork/floor junctures at the front service area. 6) Install ASSE 1022 dual check backflow preventers on each water supply line connected to non-carbonated beverage equipment. Ensure stainless or plastic backflows are utilized for any RO connections. 7) Indirectly plumb the dump sink to the floor sink and provide a 1" minimum air gap. 8) Provide a longer faucet to the three compartment sink to ensure it can reach all compartments. 9) The laminate shelving is not approved for the tea dispensers and fructose machine. Recommend wrapping the base of the cabinets with stainless steel or similar metal. An epoxy clear sealer is approved. 10) Continue caulking all sink fixtures to the walls with silicone. 11) Continue caulking all escutcheon plates and plumbing penetrations throughout the facility. 12) Provide all clean out covers. 12) Provide a separate small covered trash receptacle in the women's restroom for feminine hygiene product disposal. 13) The walls in the restrooms and food service areas must be provided with a semi-gloss finish. Eggshell is not approved. 14) Trim the weather stripping at the back exterior door to ensure the door can properly self-close. The final construction inspection is scheduled for Thursday September 7th, 2023. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F minimum. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
