Final Construction inspection conducted with owner, Toby and Superintendents.Construction completed and approved. Inspection report was emailed to owner.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with owner, Toby and Superintendents. OK to stock facility. Confirmed refrigeration is holding at or below 41°F and hot water has been provided. At the time of this inspection, this project was about 90% complete. Inspection report was emailed to Toby.Prior to final:1. Provide u-shaped toilet seat.2. Provide self closer on restroom door.3. All flooring must be smooth, expansion joints filled, and sealed well enough so that water beads on surface. Continue sealing floors throughout. Including the expansion gaps in walk-ins.4. Provide full floor sink grate in walk-in where necessary.5. Seal around all plumbing penetrations throughout (around hose bibs, above mop sink, new condensate lines, etc).6. Remove chemical dispenser from hallway.7. Provide hose bibb vacuum breakers on all hose faucets in facility.8. Restroom sink is slow to drain. All flooring must be smooth, expansion joints filled, and sealed well enough so that water beads on surface. Continue sealing floors throughout.5. Provide full floor sink grate in walk-in where necessary.6. Seal curbs in walk-in. Provide 3/8th finger cove along curbs.7. Condensate drain lines must drain to floor sink or dry well outside of facility.8. Clean all re-used surfaces.9. Remove any unnecessary wiring and trough/wrap wiring along front of walk-in.10. Remove chemical dispenser from hallway.11. Provide hose bibb water connection in mop sink area if installing chemical dispenser.12. Provide hose bibb vacuum breakers on all hose faucets in facility.13.
