Final construction inspection.Construction is complete and approved with conditions.Correct the following items:(1) Provide internal air gaps in all chemical dispensers.(2) Transition dish machine drain line to ABS, then air gap above the floor sink.(3) provide a splash guard between the hand sink and the 3-comp sink at the bar. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. E&D, class 4 10+Construction inspection was conducted with the superintendent, Eli M. Construction is at 80% completion. Some equipment are on site but not installed. Scheduled for equipment inspection on 9/14/23.The following items were discussed:1. Provide a self-closer/ self-closing hinge on exterior doors and restrooms’ doors.2. Provide splashguards on all hand sinks. Splashguards must be at the height of the faucet and length of the sink.3. Provide a reduced pressure zone (RPZ) assembly (ASSE 1013) at the water connection point of the soda carbonator. Conduits and fittings after the RPZ must be stainless steel or PEX. Do not use copper or brass alloys downstream from the backflow preventer. Provide a drain cup for the vent port of the backflow device. Drainpipe must be rigid, slope to drain and indirectly discharge to a floor sink/ floor drain with minimum 1” air gap from the finished grade.4. Provide a RPZ at the water connection point of each glass rinser. Provide drain cups for each backflow preventer. 5. Testable backflow preventers must be installed in an area that is visible, easily accessible for a person to get service the units. Minimum height is 1’ but not more than 5’ from the finished grade and cannot be inside an enclosed place or underground.6. Certify all testable backflow devices and provide reports.7. All non-carbonated drink equipment must be provided with a vented dual check valve (ASSE 1022) at the water connection point of each piece of equipment.8. All sinks must have hot and cold water. Hot water must be at 110*F and available within 45 seconds upon activation.9. All cooking equipment must be 6 inches inside the hood. Provide wings on the sides of the hood if equipment will be at the edge of the hood to contain the grease laden smoke and heat.10. Provide light color ceiling inside restrooms. Black or dark color is not acceptable.11. Provide semi-gloss paint on exposed duct works in the dinning/ bar area.12. Noted straight cove tiles inside the kitchen.
