Final Construction inspection conducted with site superintendent Michael R and Kristopher C. Construction completed and approved. ***A minor remodel permit will be created for an additional prep sink. Inspection report was emailed to superintendent and Kristopher C.Correct the following items prior to routine inspection:1) Disable the off switch for the air curtains at the bar.2) Finish installing the partitions within the restrooms.3) Provide high temperature silver colored silicone to the gap behind the wall and type 2 hood. 4) Silicone behind all equipment that weighs more than 30 lbs and caulk in all escutcheon plates. Seal the foam in the walk-in cooler (silicone or semi/high gloss pain).This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Conducted onsite visit to discuss construction related items with Michael R. Inspection report was emailed to Michael R.Okay to stock and trainCorrect/address the following items prior to final inspection on 12/14:1) Finish installing the countertop within the bar. Seal in any gaps underneath the bar and ensure all wood is painted with a semi or high gloss paint.2) Finish installing air curtains within the bar. Each air curtain must turn on via a micro switch. 3) Current double check valve backflow preventer for the misting system is not approved Provide an RPZ (ASSE 1013) backflow preventer upstream of the misting system. Provide a funnel drain below the relief valve of the RPZ and drain to a floor sink or to gravel.4) Extend the splash guards so that they meet the height of the faucet. 5) Provide brighter light bulbs within both walk-in units. Provide a cover for each light bulb.6) Wrap the condensate line within the walk-in cooler. 7) Replace the flex line for the dry rack in the bar for a hard pipe. All drain lines must be hard piped to a floor sink with a 1-inch air gap.8) Provide the C of O from the city.9) Silicone all equipment within the bar to the wall. Fill in any gaps.10) Fill in any voids around the floor sinks. Please continue with construction according to plans, stipulation letter, and inspections. Inspection report was emailed to Michael R.Correct/address the following items prior to the final inspection:1) Provide an RPZ (ASSE 1013) backflow device upstream of the soda equipment. Do not install any copper or brass downstream of the backflow device. Only PEX, plastic or stainless steel is approved. Provide a rigid drain line below the relief valve of the backflow device and drain to a floor sink.2) Provide an RPZ (ASSE 1013) upstream of the misting system. Provide a rigid drain line below the relief valve of the backflow device and drain to a floor sink.3) Test and certify all testable backflow devices and provide the test report. 4) Provide a dual check valve backflow preventer (ASSE 1022) upstream of each non-carbonated beverage equipment. 5) Ensure all chemical dispensers have an internal air gap (ASSE 1055A).6) Provide the TC of O or C of O from the city.7) Provide air curtains above the two windows within the bar.
