Cut tomatoes, and cream cheese were being stored on top of containers in flip top of prep top cooler, items measures 51-55 degrees F. Items had been stocked recently and were moved to bottom of cooler at time. Please ensure all time/temperature controlled for safety food items are maintained at 41 degrees F or below when cold holding. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with Owner. Construction completed and approved. Inspection report was emailed to Marcus SanchezPlease complete the following items by next routine inspection:1. Install splash guard between prep sink and prep refrigerator.2. Seal crevices between wall and prep sink, 3-compartment sink, and handwashing sinks with silicone.3. Repair leak on drain pipe underneath 3-compartment sink.4. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Equipment inspection conducted with Owner, Marcus S. At the time of this inspection, this project was about 98% complete. Inspection rights and inspection report was emailed to Marcus S.Please complete the following items for the Final Construction Approval Inspection:1. Ensure hot water measures at least 100 degrees F at handwashing sink, and 110 degrees F at 3-compartment and prep sinks.2. Install splash guard between prep sink and prep refrigerator.3. Paint bare wood on top of water heater pedestal with several coats of semi-gloss or high gloss paint to ensure it is smooth and easily cleanable. 4. Seal crevices between wall and prep sink, 3-compartment sink, and handwashing sinks with silicone.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. Equipment inspection conducted with Owner. At the time of this inspection, this project was about 75% complete. Inspection rights and inspection report was emailed to Marcus S.Conducted inspection with R.S. 775.Please address the following items prior to final inspection:1. Install ASSE 1013 Reduced Pressure Principle Zone Assembly (RPZ) backflow preventer between water supply line and reverse osmosis (RO) system with no copper or brass fittings downstream of preventer. Install an ASSE 1022 duel check backflow preventer between water line and coffee machine.2. Install cove base around mop sink and along wall underneath espresso counter.3. If less than 24 inches space, install splash guards between 3-compartment sink and equipment on small refrigerator, and between prep sink and prep-refrigerator.4. Provide ceiling tiles that are smooth and easily cleanable above prep area.5. Install sneeze guard in front espresso machine at least 60 inches in height.6. Seal base of vent pipe underneath espresso counter.7. Seal any gaps and crevices.8. Install air curtains above pass through windows; micro activated micro-switches properly installed over the outside/exterior entrance door(s) or window(s) openings to the exterior including roller doors, garage doors, “pass-outs”, drive-thru windows, etc. Light switches and breaker boxes must not be used in place of or to circumvent door/ window activated micro-switches. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed.
