Observed soiled cardboard in use as shelf liners in multiple areas throughout the establishment. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Discontinue the use of cardboard or single-use surfaces as shelf liners and maintain a regular cleaning frequency to prevent buildup on shelving. In the reach in cooler under the tea dispensers in front of house, observed a container of coconut milk with a date mark of 09/13. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Corrective Action: Corrected at time of inspection.

Quaternary Ammonia test strips were provided during the inspection, but the establishment is using chlorine in sanitizer buckets next to the 3-compartment sink. Provide and maintain test strips for the correct sanitizing solution maintained on site. Test strips are required for any sanitizers used with food contact surfaces. Corrective Action: Correct prior to re-inspection. Discussed with PIC why having the correct kind of test strips was necessary.

Final construction inspection.Construction is complete and approved.Correct the following items:(1) Provide a mop hanger at the mop sink.(2) Re-route the red filter drain line from the prep sink basin to the floor sink.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail.
