The Arizona Department of Health Services (ADHS) is the agency responsible for regulating the use of Kava, Kratom, or CBD in food. You must contact ADHS to obtain information on requirements related to these products. Operator indicated they are going to serve all foods listed (THC, shilajit, skullcap, three spirit elixirs) as a side car to their menu items. Operator indicated they are continuing to get information from suppliers concerning whether these items are GRAS; until they can get the correct documentation they will serve these additives as side cars.

Final construction inspection for a Class 2 10+ E&D permit was conducted with owner Sal C. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal. Equipment inspection for a Class 2 10+ E&D permit was conducted with owner Sal C. by appointment. The project is approximately 80% complete. Tailgate plan review conducted. Please ensure the following items are addressed by the final construction inspection: 1) Indirectly plumb the three compartments sink, prep sink, and ice machine to the floor sinks and provide 1" minimum air gaps above the floor sinks. 2) Provide a splash guard between the prep sink and three compartment sink. 3) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. 4) Fill all saw cuts, pitted areas, cracks, and expansion joints in the bar concrete flooring. Please provide enough clear coats to the concrete flooring to ensure all liquids bead on top of the flooring. 5) Provide a clear semi-gloss topcoat sealer to the existing painted walls behind the bar area, mop sink room, and restrooms. An epoxy matte sealer can be applied as an alternative. 6) Caulk all sink fixtures to the walls with silicone. Caulk the base of the mop sink with silicone. 7) Replace the perforated acoustical ceiling tiles above the bar area and mop sink room with smooth, washable vinyl ceiling tiles. The alternative is to flip the tiles over and provide a washable paint finish. The facility at this time will only be washing citrus products in the prep sink. Any additional produce items, such as lettuce or tomatoes, will require the facility to wash these items prior to operations. The menu at this time includes primarily pre-cooked food products that will be reheated for service. Any menu changes in the future will need to be submitted to the Department for review. Reviewed sanitizer solutions and certified food manager training. The final construction inspection is scheduled for Thursday September 7th, 2023. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
