During inspection observed a employee's boba drink without a lid and straw stored in a rack above sliced papaya & packaged meat. In a second instance observed an employees personal drink stored in a rack above packaged salsa containers. Upon request the employee drinks were relocated. Informed the person in charge to ensure personal drinks have a proper lid and straw and are stored in a designated area. Ensure to follow proper food separation at all times, to prevent cross-contamination. Corrective Action: Corrected at time of inspection.

During inspection observed stainless steel containers holding cooked menudo, cooked beans, cooked chille rellenos, cooked shredded beef & diced cabbage without a preparation date. Per the person in charge the items had been prepared and under storage for longer than 24 hours; items were properly labeled by the person in charge. Please ensure TCS food are properly labeled & used or discarded within 6 days from the prep date (7 days total). Corrective Action: Corrected at time of inspection. In the walk-in - Two containers of cooked beans found with internal temperatures of 62-*F. The beans were prepared yesterday and had begun cooling last night. PIC discarded both containers of beans. Red and green salsa also prepared yesterday found cooling on the bottom shelf in 5 gallon buckets with internal temperatures of 46-52*F. PIC will discard both buckets of salsa. Discard weights for beans and salsa provided by PIC. Reviewed cooling time frames and temperatures with PIC. Please ensure proper cooling times and temperatures are met and best cooling methods are employed. Corrective Action: Corrected at time of inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Brace all drain lines using uni-strut or equivalent means to ensure drain lines maintain air gap integrity with the floor sink (ice machine).5. Provide 3-comp will need backerod for juncture with side wall and seal.6. Seal all sink fixtures backsplash and sides (including restroom hand sinks, toilet basin, and mop area).7. Adjust the metered faucet in men’s restroom so that it stays on 15 seconds are longer without reactivation.8. Ensure drive through window is self-closing (needs to be adjusted).9. Seal the hood vent and juncture between wall using heat resistant silicone (silver).10. Seal all holes, voids, gaps, cracks, and penetrations (walk-in, electrical, and plumbing) as discussed.11.
