Final construction inspection for a Class 2 0-9 E&D permit was conducted with superintendent Randon H. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection for a Class 2 0-9 E&D permit was conducted with PR supervisor by appointment. The project is approximately 80% complete at time of inspection. Please ensure the following items are addressed by the completion of the project: 1) Provide an ASSE 1013 reduced pressure backflow preventer upstream of the soda carbonator. Ensure no copper or copper alloys are downstream of the backflow. Certify and provide a test report. 2) Observed the tile base in the kitchen and restrooms topset with a small 90' angle. Please provide a 3/8th minimum finger or tool cove with silicone grout. 3) Provide a 1/4" or 1/2" gravel path for mop buckets to be easily moved to the restroom area from the mop sink area. 4) Caulk-in all wall penetrations and escutcheon plates in the restrooms, as needed. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. A final construction inspection is scheduled for Monday December 11th, 2023. MCESD inspection report was delivered via email. Plumbing inspection for a Class 2 0-9 E&D permit was conducted with superintendent Randon H. by appointment. The project is approximately 40% complete at time of inspection. Please ensure the following items are addressed by the completion of the project: 1) The existing ice machine located outdoors is not rated for outdoor use. Please replace with a NAMA approved outdoor ice machine. 2) Provide FRP around the mop sink walls. The FRP must be installed from cove base to 12" above the mop sink faucet. 3) Provide an ASSE 1013 reduced pressure backflow preventer upstream of the soda carbonator. Ensure no copper or copper alloys are downstream of the backflow. Certify and provide a test report. Proposed water tank system will be reviewed by the Water and Waste division for compliance. Equipment inspection is scheduled for Tuesday November 28th, 2023. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
