Person in charge must be a CFPM and available at the site during all operational hours. Food handlers must have food handlers’ certificate. Maintain all managers and food handlers’ certification on site for inspection purpose. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Construction inspection was conducted with the owner, Leslie C. and installers of the combi-oven to discuss drainpipe and backflow preventer of the ovens. Installers will provide minimum 1” air gap from the finished floor for the drainpipe of the combi-ovens. Owner will contact a certified backflow testing company to install the reduced pressure zone assembly at the water connection point of the ovens. Ensure the backflow preventer must be certified after installation. Food cateringPlumbing inspection was conducted with the owners, Chad and Leslie. Plumbing stub out had been completed. The person in charge must be a certified food protection manager (CFPM) and available at the site during hours of operation. All food handlers must have food handlers’ certification. Maintain permit, license, and certificates at the site for inspection purpose.Please address the following items prior to the next inspection:1. Provide a reduced pressure zone assembly (RPZ) ASSE 1013 standard at the water connection point of the combi-ovens. Provide a drain cup for the vent port of the device. Certify the backflow preventer and provide report.2. Repair/ replace the atmospheric vacuum breaker (AVB) at the mop sink’s faucet. The cap of the AVB is missing also. Please provide.3. Elevate the drainpipe from the oven to 6 inches off the floor. Brace the drainpipe along the wall with metal brackets, slope to drain at 1/4” per foot and ensure the end of the drainpipe must be at least 1” from the finished floor. Noted the drainpipe is inside the floor sink.4. Provide elbows at turns of drainpipes and at the end of the discharge pipe.5. Provide self-closing doors or air curtains at the entrances of the restrooms.6. All exterior and restrooms’ doors must be self-closing.7. Provide door sweeps and weather stripping at all exterior doors to prevent entrance of pests.8. Caulk and seal all gaps, cracks and penetrations throughout. Noted there is an outdoor walk-in cooler on premise. Per owner, the unit had not been working and they may not be repaired by the final construction inspection. • For the walk-in unit:- Anchor the gaps between floor panels of the walk-in. They are popping out.- Caulk and seal between walls, ceiling, and floor panels, holes, conduits, and penetrations inside the unit.- Provide overhead cover from the back door to the walk-in cooler door.No County legal action will result from this inspection.Emailed the report to the owner.
