Inspector's notes

Observed hand sink in back room with multiple things in basin of sink. (Tools, scrubber) Discussed only using a hand sink for hand washing and no other purpose. Observed sink in back room without soap. Discussed always having hand soap and towels at every hand sink at all times. PIC stated they do not use sink to wash hands and only use the one up front. Informed PIC that sink in back should be used when 3 comp sink is in use. Employees cannot handle dirty dishes and then clean dishes without washing hands first. Corrective Action: Corrected at time of inspection. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. PIC will provide soap for hand sink.

Final construction inspection.Construction is complete and approved with conditions.Correct the following items:(1) Provide 3/8 radius coved base in the restroom and back kitchen area.(2) Provide dome strainers in all floor sinks.(3) Install a self closing mechanism on the restroom door.(4) Paint the exposed wood at the water heater and front service area floor sink.(5) Caulk the back kitchen hand sink to the wall.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov.
