PRIORITY VIOLATION-3-501.16(A)(2) and (B) - Priority: Time/Temperature Control for Safety Food, Cold Holding. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. According to the PIC all TCS foods were placed in the salad station refrigerator an hour ago. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection.

In the meat walk in refrigerator observed two open, sliced, uncured deli ham chubs. One chub had a 1/15 date mark and the other had a 1/31 date mark. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Discussed date marking requirements and reviewed date marking requirements for ready to eat TCS foods that had been frozen and then thawed. Corrective Action: Corrected at time of inspection.

OC re-inspection for a Class 4 10+ E&D permit was conducted with sous chef Roman B. by appointment. The establishment was open and operating at time of inspection. Please address the following items prior to the next OC re-inspection within 10 days: 1) Remove the hub drain from the three compartment sink and replumb the sink directly to the system. Provide an indirect connection by installing an open vent pipe on the three sink drain line. 2) Install ASSE 1020 spill proof pressure vacuum breakers upstream of the sprayer hose at the large warewash machine and the spray hose at the ice machine area. Certify and provide test reports. 3) Install ASSE 1022 dual checks at all non-carbonated beverage equipment. 4) Please install condensate pumps at the exterior of the walk-in coolers for the condensate drain lines. Please pump the condensate to the nearest floor sink. Ensure audible alarms are provided on the condensate pumps. 5) Switch the hot and cold water knobs at the handwash sink near the large warewash machine. The hot must be on the left. 6) Replace the perforated acoustical ceiling tiles in the dry storage room, as needed, with smooth, washable vinyl ceiling tiles. 7) Increase the lighting in the beer and wine cooler unit. 10 foot candles is required throughout all areas. The facility will be utilizing the handwash sink at the beverage station for the large ice machines. The handwash sink is not visible from this area. If any handwashing violations are observed at the ice machine area then an additional handwash sink will be required. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. PRIORITY VIOLATION-5-203.14, P: Backflow Prevention Device, When Required -------- Observed a spray hose at the warewash trough area. Please provide an ASSE 1020 spill proof pressure vacuum breaker for the hose system. Certify and provide a test report. Please disconnect the trough system from the plumbing system since it is no longer utilized. Observed hose spayer with a shut off at the large ice machine area. Please provide an ASSE 1020 spill proof pressure vacuum breaker. All hose connections that have a shut-off must have a testable backflow device designed for continuous pressure. Atmospheric vacuum breakers are not approved. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained ------- Observed multiple indirect drain lines sitting within the floor rim of the floor sinks. Please elevate or trim the drain lines to provide 1" minimum air gaps above the floor sinks. Observed chemical dispensers with ASSE 1055B flex gaps. Please provide ASSE 1055A internal air gaps to the chemical units. Observed the large coffee brewers in the beverage station area without backflow preventers. Please provide ASSE 1022 dual checks on the water lines. Observed the hot water knob installed on the cold side of the handwash sink located near the warewashing area. The hot water knob must be on the left side. All plumbing systems must be maintained in good repair to prevent cross connections and to ensure they operate as designed. PRIORITY VIOLATION-5-402.11, P: Backflow Prevention ------- Observed a hub drain installed below the three compartment sink. The hub drain isn't large enough to handle the waste from the compartments. Please replumb the three sink directly to the grease interceptor. Please install an open vent pipe on the drain line to create an indirect connection. Please ensure the drain line is 1" minimum below the compartments. Provide a studor vent for any odors. All equipment and sink fixtures that contact food and food contact surfaces must be indirectly plumbed to prevent a cross connection. Core-5-403.12, C: Other Liquid Wastes and Rainwater -------- Observed the condensate drains for the walk-in coolers draining onto the floor to a nearby floor drain. Please install condensate pumps with audible alarms at the exterior of the walk-in cooler and pump the condensate to an approved floor sink. All condensate must be properly plumbed to a floor sink.
