Provide a stainless steel side splash between the back hand sink and the adjacent three compartment sink.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. There is a chemical dispenser system with shutoff valves connected to the mop sink faucet. There can be no chemical dispensers or shutoff valves connected to mop sink faucets. Provide dedicated water line for the chemical dispensers or remove them completely.2. There is a chemical dispenser without a proper air-gap installed at the three compartment sink. Provide a chemical dispenser with a proper air-gap at this location or remove it completely.3. A brass fitting is connected on the downstream side of the RPZ backflow prevention device for the water supply to the soda carbonators. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. Remove the brass fitting.4. There are flex drain pipe from the ice machine extending into the floor sink near the machine. Provide hard pipe drain lines such as PVC that empty into the nearby floor sink with minimum 1 in. air-gaps between the ends of the pipes and the flood rim of the floor sink.5. Repair the hot and cold water at the restroom hand sinks.

Handwashing sinks must be centrally located, visible, and directly accessible within 25-feet of all food/beverage preparation, food/beverage dispensing, ware washing and similar workstation areas. This includes any food or beverage preparation conducted in the “Party Prep Room.” Barriers must not physically and/or operationally obstruct the hand washing facility.2. Provide a sneeze guard with side panels for protection of all exposed food along customer serving counters. All sneeze guards are approved on a case-by-case basis and require final on-site approval.3. Provide the 80-gallon capacity water heater for the kitchen and the 40-gallon capacity water heater for the restroom as indicated on your submitted plans to meet the minimum hot water demands of the fixtures in the facility.4. Floor and wall junctures must be properly coved, enclosed and/or sealed. (FC §6-201.13) Provide an approved smooth, cove base (3/8” radius or greater) at wall and floor junctures and equipment bases throughout the establishment including dry storage areas, restrooms, cabinet bases, and similar areas.5. Exposed, rough, unfinished wood is not an approved finish. Paint/seal wood located under countertops and bars. Paint or seal any wood exposed from cabinet cutouts or conduit penetrations. Laminate countertops must not be installed in food or moisture areas, including customer self-service areas, permitted restrooms or hand wash areas.6. Ensure all restrooms are completely enclosed and provided with tight-fitting and self-closing doors.7. Each urinal must occupy a separate area with walls or partitions to provide privacy. 8. Provide an enclosure with doors for the washer and dryer installed in the kitchen area.9. Provide an additional water line, with an angle-stop or ball valve, at the mop sink/ utility area to supply chemical dispensers. Do not use the mop sink faucet. Y-Valves and downstream shut-off devices are not permitted on mop sink faucets or chemical dispensers.
