Final Construction inspection conducted with owner Amanda A. Construction completed and approved. ***Bins that do not self close must only be used for pre-packaged candies. ***Permit is approved for repackaging of bulk candies (Non-TCS foods).Inspection report was emailed to Amanda A.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with Amanda A At the time of this inspection, this project was about 75% complete. Inspection report was emailed to Amanda A.Address the following items prior to the final inspection:1) Ensure all bins in the retail area self-close.2) Provide the following information for bulk food: The manufacture’s or processor’s label that was provided with the food; or a card, sign or other method of notification that includes this information.3) Provide labels to all packaged candy done within the establishment. Provide the common name of the food, if applicable ingredients in descending order, net quantity of contents, the name and place of business packaging the product (i.e. Mad Candy), and food allergens.4) Provide semi/high gloss paint in the restroom, dry storage and kitchen.5) Provide escutcheon plates or caulk around all pipe penetrations throughout.6) Provide a smooth and easily cleanable floor within the restroom and kitchen. Provide a water bead sealant over the concreate flooring. Water must completely bead up.7) Provide light shields within the kitchen and dry goods room.8) Ensure the restroom and back door self close.9) Provide a food manager certification from an accredited company.Please continue with construction according to plans, stipulation letter, and inspections.
