*SECOND CONSECUTIVE VIOLATION* Observed missing caulk around hand sink to left of three compartment sink in the warewash area. EQUIPMENT that is fixed because it is not EASILY MOVABLE shall be installed so that it is SEALED to adjoining EQUIPMENT or walls, if the EQUIPMENT is exposed to spillage or seepage. Correct prior to next routine inspection. 4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: There is excessive ice and frost buildup in both walk in freezers under the fan areas and on shelving and packaged product. PIC stated that the units would be scheduled for maintenance/repair. Ensure that the equipment is maintained in good repair. Corrective Action: Correct prior to next routine inspection.

Observed at time of inspection 77*F-80*F with inspector thermometer temperature of hot water at 5 minutes. Notified Person In Charge (PIC). PIC will have maintenance review hot water issues. Hand sink by main walk in unit does not have water source. Not able to get to 100*F. This hand sink needs to be connected to water and hot water at 100*F. ========100°F through a mixing valve or combination faucet at each hand sink. Corrective Action: Correct prior to re-inspection. Final Construction. Walked with owners and contractor. Food Processor Class 4.Correct the following items within 2 weeks:1) Adjust drain line from 4-compartment sink in production area so that drain aligns fully over floor sink with a 1” air gap to prevent water from skirting out onto floor.2) Ensure all sinks and ware washing tables are secured to wall and sealed with caulk or silicone.3) Floor for large walk-in freezer must be level, all saw-cuts filled in, and sealed.4) Remove all protective plastic covering from all walk-in panels, including ceilings.5) Provide cam covers where missing in all walk-in units.6) Install rubber cove base were missing in production area.7) Cove base throughout rest of establishment has been siliconed in poorly; it is not smooth or easy to clean. Either cut all excess silicone away so that a true smooth cove base is provided, or apply another cove trim over existing base.8) Seal all small holes in walls with caulk or silicone. This includes beams in production area, walls in hallway to chemicals, walls in employee break area/office area.9) Provide door sweeps to both doors to the exterior from maintenance room, and weather stripping on latch side of door to the south, so that no light is visible around either door.10) Provide a split toilet seat to right restroom in employee area.11) Ownership states they are FDA registered; please provide registration number.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2.
